We have shared the story of Emily’s journey after she broke her leg over the past few months. Emily’s rollercoaster of a recovery journey presented several challenges for both her and her son Ben. Hospital Care

Emily’s broken leg and ensuing surgery proved more complicated than she and Ben were prepared to handle. When our care manager met them in the hospital, she began organizing Emily’s care to make the process easier for Ben and Emily by:

  • Explaining the importance and role of physical and occupational therapy
  • Suggesting ways to handle frustration at new limitations
  • Preparing Emily’s home for her new mobility needs

Rehabilitation and Home Life

Once Emily was discharged from the hospital, the recovery process was just beginning. There were so many little details of rehab and home care that needed to be coordinated. The care manager stepped up to assist with each part, including:

  • Researching and coordinating entry to the right rehab facility for Emily’s situation
  • Arranging in-home support for meals, housekeeping, and companionship
  • Introducing new ways to participate in social life
  • Suggesting solutions for physical discomfort
  • Organizing admission to an art therapy program

Regaining Confidence

After Emily’s misalignment and second surgery, she relied on the care manager more than ever to help her get back to the life she loved. Working with Emily’s changing abilities, concerns, and needs, the care manager:

  • Communicated Emily’s situation to the rehab team to adjust treatment for her specific circumstances
  • Introduced Emily to new perspectives on her recovery to give her encouragement
  • Constructed a step-by-step plan to restore Emily’s confidence in her old hobbies

Into the Future

After such a small event led to an arduous recovery, Emily wanted to be more prepared to overcome aging challenges in the future. The care manager worked with Ben and Emily to make the decisions that would prepare Emily to age with confidence:

  • Created a care plan tailored to Emily’s specific circumstances
  • Brought forward situations that might require additional care in the future and helped Emily decide on her aging care preferences
  • Provided nutritional and exercise changes that would help prevent future incidents
  • Formed a plan of action to be implemented in case of a medical emergency
