As part of celebrating the month of May as Aging Life Care Month, we would like help educate our visitors on what it is that Aging Life Care Professionals, also known as Geriatric Care Managers, do to help aging persons and their involved family members to plan for care and solve aging challenges.
An Aging Life Care Manager, or Geriatric Care Manager, is a trained professional who maintains a holistic, client-centered approach towards care management. As care managers, we do everything in our power to advocate for what is truly best for your loved one, and we do so with access to a wide variety of knowledge and resources that most people do not.
If the knowledge of what an Aging Life Care Manager is foreign to you, you are not alone. The services that would be available through our practice start with a detailed assessment of the current care situation for an aging or disabled person. From there, a care plan is created to determine services that would help the aging person remain independent, meet complex healthcare needs, manage dementia care, and provide education and comfort to the family.
